Please fill out and submit this form if you experience any technical difficulties that are non-emergency in nature when operating a videoconference classroom. For example, please use this form if the fax machine needs maintenance or a student microphone is not working.

For emergency issues, please refer to the "Interactive Video Manual" that is located near the telephone in each ITV classroom.
